Blank applications and Physical forms for Camp Courageous of Iowa are not available on this web site. You will be sent an application and physical once you are schedule for camp or you can log onto the camp's Online Paperwork system and fill out the application electronically. You can also view/print a physical form for a camper.

Release Form for Camp Courageous
You can download a Camp Courageous release form. Every camper attending Camp Courageous must sign a release.

HIPPA Form
If you have not signed a Health Information Privacty Policy (HIPPA) for Camp Courageous of Iowa then you can download, print, and sign it. Please bring this form with you. If you do not have a signed form when you arrive at camp you'll be asked to fill one out at check-in.
