APPLICATION FOR EMPLOYMENT, VOLUNTEERS, INTERNS
To apply for employment or volunteer work at Camp Courageous you will need to fill-out and submit an Employment Application. The application is a PDF Form that you can download. Once downloaded you can:
- RECOMMENDED: fill-out the form electronically (type your information right into the PDF form fields), digitially sign it, and then submit it back to the camp.
- Fill-out the form electronically, print the form on your own printer, sign it, and then mail the form back to the camp via postal mail or fax.

ABOUT DIGITAL SIGNATURES
If you elect to sign your employment application with a digital signature the Adobe Reader software will take you through the process step-by-step. You are creating a digital security certificate for yourself with your own private code number (PIN) and attaching it to the form. This digital signature is a legally binding signature, just as if you signed it manually with a pen.

Use this form only if you have been offered a position at Camp Courageous and have accepted. Download this form and provide it to your physician at your next physical exam. Please have all parts filled out and signed by your primary medical physican. Mail this form to the camp prior to your arrival for work.
